Dr David Swart Dentistry
Terms and Conditions
These terms and conditions apply to all patients of Dr David Swart Dentistry, and to the parents or guardians of patients under the age of 18. By completing a new patient form, accepting treatment, or entering our premises, you confirm that you have read and agree to these terms, together with our POPI Framework and Policy.
1. Payment of Account
The patient, or where applicable their parent or guardian, is responsible for settling all account balances.
By accepting these terms when completing a new patient form, you accept responsibility for the fees associated with the initial appointment. Any quotes agreed to afterwards will likewise remain the patient’s responsibility for payment.
2. Missed Appointments and Late Cancellations
Appointment time is reserved specifically for you. Please let us know as early as possible if you are unable to attend.
Dr David Swart Dentistry reserves the right to charge a fee for missed appointments and for appointments cancelled at short notice.
3. Medical and Dental History
To provide safe and effective care, patients must give a complete medical and dental history, including any known conditions, allergies and current medication. Please inform us promptly of any changes to your health during treatment.
4. Emergencies
If you experience an emergency, such as significant pain, please contact the practice during normal operating hours. After-hours contact options, including WhatsApp and SMS, are available for urgent enquiries. Additional fees may apply for after-hours assistance or emergency appointments.
5. Sharing Information with Other Practitioners
For comprehensive treatment, some patient records may need to be shared with other healthcare practitioners, such as oral surgeons or specialists. By agreeing to treatment, you consent to the sharing of relevant information for the purpose of coordinated care, in line with the Protection of Personal Information Act (POPIA).
6. Discontinuation of Treatment
Dr David Swart Dentistry reserves the right to discontinue treatment in certain circumstances, including but not limited to non-payment, non-cooperation or repeated missed appointments. In such cases, the patient or guardian will be informed. Discontinuation of treatment does not entitle the patient to a refund of fees already paid.
7. Oral Hygiene
Good oral hygiene is essential to the success of your treatment. Poor hygiene, such as not brushing and flossing properly, can lead to complications. Patients are asked to follow all hygiene recommendations given by the practice.
